T450s no sound - randomly witches sound output to "2nd Output" (PICS)

I having a strange problem, which I suspect might be due to "Deep Sleep Mode" although I will have to test it a couple more times.
I noticed time to time my sound output switches to "Realtek HD Audio 2nd output" without any switching on my part.


I can see the sound indicator (level) going up and down when I playing and music or video file, but I am not sure where it's sending that sound.

When I switch back to Speaker/HP by hitting that little arrow, I can get the system sounds (ding when adjusting volume) but nothing else, rest still seems to be routed to "2nd output".

Does anyone have an idea what could be the reason for this? It happened again, even after a restart, the moment I put it to sleep, upon wake up, sound is gone..
